An otherwise healthy, 34-year-old woman was admitted to the medical floor of the hospital with complaints of shortness of breath (SOB). Her admission diagnosis was pneumonia. Despite appropriate treatment, her respiratory status worsened and she required transfer to the Intensive Care Unit (ICU). The patient was suffering from acute respiratory distress syndrome (ARDS) secondary to sepsis syndrome with infection from Influenza B.

We may actually be doing a disservice to patients by adjusting the ventilator based on high plateau pressure and increasing the chances of lung damage by continually opening and collapsing the alveoli. Using an esophageal balloon, we can separate the two components of respiratory system compliance and adjust the ventilator based on the estimated transpulmonary pressure.
